Nomination Form
Thank you for your interest in nominating a fellow Islander for the Order of PEI!
The nomination form is available through the link below as an online webform, and through the links at the side of the page (in both English and French) as a PDF to print out and fill in by hand.
Nomination form requirements
If you have a deserving Islander in mind whom you'd like the Advisory Council to consider for investiture in the Order, you'll need to gather the following information and documents before you start filling out the form:
- your contact information (you are the nominator)
- name, address, phone number, and email
- your county of residence, preferred language (English or French), and relationship with the nominee
- your nominee's contact information
- name, address, and a current phone number (email address optional)
- the nominee's county of residence and preferred language (English or French)
- your reason(s) for nominating the person
- no more than 500 words (two single-spaced, letter-sized pages) explaining your personal understanding or connection with the nominee's unique or significant contribution(s)
- biographical summary for the nominee
- no more than 500 words (two single-spaced, letter-sized pages) providing some biographical details about your nominee, as well as a summary of their achievements or recognitions, if any
- three letters of support for your nominee from three different people, along with their contact information
- name, address, and either a phone number or an active email address
- relationship with the nominee
If you are filling out the online webform, you'll need electronic copies of the documents to load into the form. Microsoft Word files (.doc, .docx), image files (.jpg, .png), and PDFs are all formats accepted by the form.
Guidelines for a successful nomination
Your nomination package is the only information presented to the Advisory Council for consideration on behalf of your nominee, so here are some tips to assist with a smooth nomination:
Please ensure:
- every effort is made to convey the importance and impact of your nominee;
- all questions are answered, as an incomplete package may result in your nominee not being put forward for consideration;
- all attachments are included with your nomination; and
- your nomination package is received before the closing date.
Please do not:
- tell the nominee about the nomination;
- submit a résumé or cv for the biographical summary;
- submit more material than the maximum of 10 single-sided, letter-sized pages allowed by the nomination form;
- contact OPEI Advisory Council members to advocate for your nominee; or
- organize additional nominations or letters of support for your nominee.
Tips for using the webform
- use the navigation buttons at the bottom of the screen to move between pages so you don't lose progress;
- fields marked with a red asterix are required and have to be completed to move on in the form;
- alerts for missing information (if any) appear at the top of the page;
- the form may time out after a long period of inactivity, so make sure you have all the information and documents listed above ready to go;
- we recommend having electronic copies of documents ready to load to the form;
- you can copy and paste information into sections with large text fields;
- you can upload pictures of documents (we may contact you for copies if the image isn't clear);
- you won't receive a copy of your completed form but there is an option to print the completed version from the review screen before you submit it; and
- you'll receive a confirmation screen that your form was submitted.
